Please read this Medical Waiver & Liability Release carefully before participating in any classes, sessions, or services.

This waiver applies to all classes, sessions, and services offered by the Company.

1. Medical Fitness to Participate

I confirm that I am physically fit and have no medical condition, injury, illness, or impairment that would prevent me from safely participating in physical exercise, reformer Pilates, or wellness-related activities.

I acknowledge that it is my responsibility to consult with a qualified medical professional prior to participation if I have any concerns regarding my health, fitness, or ability to safely take part.

2. Disclosure of Medical Conditions

I agree to inform the Company and/or the instructor prior to participation of any injury, medical condition, pregnancy, or physical limitation that may affect my ability to safely participate.

Failure to disclose relevant information is done at my own risk.
